Invitation to participate

The second Eastern Metro Regional Disability Carers Forum is just around the corner and we would like to invite you to participate in the forum showcasing your services to cares, families and people with disabilities. Please download the form below for application details:

Forum Registration

Registration is available via this downloadable form. Either print and mail your details to RIDE, or email the completed document back to us. Please note that registrations are due by 8th October.

Directions to the conference venue are also included wih the Registration form, however if you have any other questions about getting to the Hawthorn Town Hall by public transport, you can also contact Metlink for assistance on 131 168 (Phone) or 03 9619 2727 (TTY).
